Waiver and Release/ 放棄追訴同意書:
I voluntarily participate in activities organized by the Houston Taiwan Institute for SEnior Citiznes and understand that such activities may involve riskes, including falls, traffic accidents, or other unforeseen injuries.
本人自願參加休士頓臺灣松年學院所主辦之各項活動，並了解活動可能存在跌倒、交通意外或其他不可預期之風險
I agree to take responsibility for my own safety, health, and conduct and to assume allrisks associated with my participation. In the event of injury, accident, or loss, I agree not to make any claim or pursue legal action against the organizers, representatives, staff, or volunteers.
本人同意自行注意安全、健康及個人行為，並自行承擔參與活動之相關風險。如發生意外、傷害或損失，本人同意不向主辦單位、負責人、工作人員或志工提出索賠或法律追訴。
This agreement is effective upon aceptance and reminas valid for three (3) years. 本同意書自同意日起生效，有效期三年。
